Come join us at Alameda County Library to enroll or get information on affordable Health Insurance through Covered California or Medi-Cal. In this free event Covered CA certified enrollment counselor and outreach educator will assist you individually. Please bring the following for enrollment. Open Enrollment starts Nov. 15, 2014, and ends Feb. 15, 2015.

• Social Security # and date of birth for all applicants
• Proof of immigration status (Naturalization certification/Green Card) if applicable
• Proof of residence (Photo ID, Utility bill)
• Proof of household income (Pay stubs, Tax forms)
• Health Insurance info if you currently have one
