The mission of the Conversations: Health and Treatments CHAT project is to empower adults of all ages— those who are healthy and those who are not— with information about advance care planning, allowing for more open communication with their families and clinicians about patient-centered medical decision-making for themselves and their loved ones should they be faced with a serious, life threatening or end-of-life situation.

For many people, advance care planning can be a scary and confusing process; however, advance directives (including health care proxies, living wills and DNRs) are powerful tools that can help demystify an emotionally charged planning process. An informed patient is an engaged patient, and by providing free, confidential legal counseling, the CHAT project seeks to (1) help people complete critical advance care planning documentation and (2) prepare these individuals for important discussions with their loved ones and clinicians through the use of these documents.

The CHAT project is a partnership between the Gitenstein Institute for Health Law and Policy at the Maurice A. Maria Carney MD is an internist, geriatrician and palliative care physician with public health experience. She is the first known Geriatrician to serve as commissioner of a health department in New York State. She is currently the Chief, Division of Geriatric and Palliative Medicine in the Northwell Department of Medicine and Hofstra Northwell School of Medicine.

Adam Kahn is a Senior Fellow for the Gitenstein Institute for Health Law and Policy at Hofstra University and an associate attorney with the law firm of Genser Dubow Genser & Cona, LLP. He was a founding member of the Hofstra Elder Law Society.
