2. Social security card or ITIN for taxpayer, spouse and all dependents
3. W-2s, 1099s, 1098s and any other tax forms that have been received.
4. Voided check for direct deposit
5. Copy of last year’s tax return, if available
6. Landlord’s name, address and phone number to claim California renter's credit.

    Celebrate the life of Dr. Seuss at the library! We'll have a few different craft projects based on some of Dr. Seuss' most famous stories, and we'll have a raffle for a few fun Dr. Seuss-themed prizes. Projects will be aimed at elementary school-aged children. Children ages 5 and under will need an adult helper for all crafts. Due to space limitations, we will be limiting class size to 50 children. Free tickets will be passed out at the Children's Desk to the first 50 children in line, beginning at 10:45 am the day of the program. The program will begin promptly at 11 am and no new participants will be admitted after 11:15 am.

De Young Museum docent and lecturer, Marsha Holm, will discuss a selection of paintings by the leading artists of the Dutch Golden Age. On exhibit at the de Young Museum in San Francisco from January 26 - June 2, 2013, this once-in-a-lifetime exhibition, includes Vermeer’s timeless masterpiece, and is made possible only because of the current renovations and expansion of the Royal Picture Gallery Mauritshuis in The Hague, Netherlands. Ms. Holm will include a slide presentation with her lecture.

As a popular speaker with Fremont audiences, Marsha Holm has been a docent with the San Francisco museums since 1979. In addition to giving tours in all areas of the museum's collections, she has served in several administrative capacities, including Chair of the latest training session for new docents, a three-year program. She has also assisted in training docents at Blackhawk Museum, Oakland Museum and the San Jose Museum of Art.

Please note that this special presentation will be held at a time when the library is closed.  Enter the door facing the parking lot to the right of the main entrance.

Chinese Medicine Day was first celebrated in 1929 in mainland China in the month of March, and has since become a yearly tradition. The Chinese medical community of Northern California has been celebrating this important day annually for the past thirty-five years in the San Francisco Bay Area.

In celebration of the 2013 Chinese Medicine Day, the Fremont Main Library is co-sponsoring a FREE health evaluation event with United California Practitioners of Chinese Medicine (UCPCM) and Citizens For Better Community (CBC) in the Fukaya Room, 1PM to 4 PM, on Saturday, March 30, 2013. In addition to free consultation (no needles), the Chinese medical doctors will offer free massages, time permitting.

There will also be two bilingual health seminars conducted in English and Mandarin:

3. W-2s, 1099s, 1098s and any other tax forms that have been received.
4. Voided check for direct deposit
5. Copy of last year’s tax return, if available
6. Landlord’s name, address and phone number to claim California renter's credit.
