Presented by South Nassau Communities Hospital, this six session program will provide you with everything you need to know about diabetes, and how you can help manage it for yourself, or your loved one. The Diabetes Self-Management Program is a free six-week workshop for people with type 2 diabetes and pre-diabetes.  Topics include exercise, monitoring blood sugar, healthy eating, preventing low blood sugar, skin and foot care, medications and stress/relaxation.
