The East Bay Conversation Project will be offering a FREE presentation about Advance Care Planning at the San Lorenzo Library Learning Center. Learn why it’s so important, what resources are available to you, and how to actually PLAN! It’s easier than you may think.
Understand what an advance health care directive does and how to complete one, things to consider when choosing a health care agent, and tips for having “The Conversation” with loved ones. You’ll receive a packet of information to take with you to review and share with friends and family.
