Isolation to Connection Info Session
The Isolation to Connection program helps link seniors to an array of community-based services such as home delivered meals, paratransit transportation, ongoing case management programs, home based medical services, mental health counseling, and friendly visiting. Together the senior and the Social Worker will establish a rapport, assess needs, make referrals, and follow up to ensure services are in place while providing a warm connection based on each person’s need. Accessing services can be overwhelming but when there is a caring professional providing support at each step of the way, it becomes more bearable. Together the goal is to help the seniors continue to live a meaningful life in their home and community.

Registration Required.
