Bring all the following items to your tax appointment: 

  1. 2018 Federal and State Tax Forms: if you filed OR Call the IRS AT 1-800-829-1040 for a “IRS Wage & Income Transcript” documenting your income if this is your first application.
  2. Proof of 2018 Income: 1099 FORMS: Social Security, Pension, Interest, Dividends, Capital Gains, Miscellaneous Reported Income.
  3. W-2 for Salary or Wages
  4. 12/31/2018 bank/broker statements from IRAs, Keogh, 401K, 403B accounts
  5. Other Income: Bring documentation to prove: Rental Income
  6. Income from other family members who live with you Workman’s compensation, unemployment, alimony Income from a trust
  7. Proof of Medical Dental & Drug Expenses that are paid out-of-pocket: Print-out from your pharmacy or doctor if possible EOB statement forms Letters or statements showing how much you paid for drug insurance, supplementary insurance, dental, eyeglasses No cancelled checks!
  8. Bring a copy of last year's property tax form filed 
  9. For first-time filers - you must also bring:

  2. Proof of 2018 Income: 1099 FORMS: Social Security, Pension, Interest, Dividends, Capital Gains, Miscellaneous Reported Income.
  3. W-2 for Salary or Wages
  4. 12/31/2018 bank/broker statements from IRAs, Keogh, 401K, 403B accounts
  5. Other Income: Bring documentation to prove: Rental Income
